Paul a very good blog.  Rapamycin, Acarbose and a new one for me Canagliflozin.  This might be better than Metformin.  What was interesting was that Acarbose lowered HbA1c which is not easy to do.  Progress marches on little by little.  Richard Miller said it right about all those Dr.'s making a lot of money on NAD precursors and ripping people off.  That includes Sinclair.  Sinclair had already ripped off the drug companies with Resveratrol.  No NR/NMN found in tissues where it is needed.

    • Van Van - all these medicines are easily and inexpensively available via vendors on indiamart.com and I"ve had no problems getting them delivered in California. Cost of Acarbose 50mg tablets is typically around $.25/50mg tablet (in quantity) - you typically take about 100mg just before each meal,, and Canagliflozin and empagliflozin (same type of drug, newer) are in the $.8 for a 10mg tablet, which is taken once per day. Review the side effect profiles of these medicines.. you can get a lot of gas from Acarbose, and the Xflozin drugs seem to increase risk of UTIs significantly... not sure if that is an issue for males as well as females.

Hi Brin, I live in Spain and can get all of these at very cheap prices without an Rx.  Have not tried Canagliflozin yet, but wonder if it is better than Metformin.  I have had very few problems with Metformin.

    • Van In the NIH ITP Lifespan studies it seems that Acarbose and Canagliflozin were much better results that metformin... I took Metformin before, but have moved over just based on the data.  Ultimately I want to gather more personal information on my own body's reaction by using a continuous glucose monitor and regular phenoage tracking.

Brin Chikovski Which one would you recommend to keep glucose low. Metformin has successfully helped me control weight by cutting cravings for carbs. Any side effects? Thanks I also plug my lab numbers in Phenoage calculator. https://www.oliverzolman.com/phenoage-calculator

    • Van I'm just going by the NIH ITP Study results - I'm just a biohacker like you, so I can't recommend anything - I'm using Rap and Acarbose and may try Canagliflozin once I have a continuous glucose monitor:

      Metformin: 4% lifespan improvement

      Acarbose: 16%, 17% lifespan improvement

      Canagliflozin:14% lifespan improvement

Brin Chikovski acarbose dose 3 times a day ? Canagliflozin once daily easier...?

    • Al m Yes - I agree, and probably more cost-effective.  Or, Perhaps a balance of the two (Or alternating) to minimize side effect risk?

      Al m You can take 100 mg x 3 = 300 daily also just like Acarbose. https://www.drugs.com/dosage/canagliflozin.html

Brin Chikovski Just checked price of Arcarbose, 40 x 50 mg = $2.  Canagliflozin, $2 per tablet, WOW.

    • Van Van - how does it work there... do you need a prescription from a doctor or can you just walk in and buy it (Acarbose) over the counter?

Brin Chikovski Just walk in and buy.  It is the drug lobby in US that makes so many drugs Rx only, not that problem in Europe.  The governments of each country pay for and negotiate for the drugs for entire country.  Drug and Health care delivery is not a business in EU

I just started taking Arcarbose instead of Metformin.  My plan was to take 100 mg Arcarbose with meals 3 X's a day.  The reason that I was going to try and change was the Arcarbose seem to do a better job in control glucose and extending lifespan.  It also had a bigger impact on HbA1c which for me was hard to get lower.  Well I took 1,000 mg Metformin at noon and 3 hours later had a meal.  Took 100 mg Arcarbose with that meal and 30 minutes later felt bloated and just over all bad.  Took nothing else until next day.  Told myself to not take them together.  Next day took 50 mg instead of 100 with my meal and felt great no problems.  So that is where I am at at this moment.  Read on the web that it takes awhile to let your body adjust to Arcarbose just like it did with Metformin.  WIll report back when I have more data.  I would not take both together.  I took Metformin to control Glucose and just like you I was pre-diabetes.  Metformin schedule was 500 mg morning, 1000 mg 12-2pm., and 500 mg at bedtime.  Really reduced my cravings for carbs, and helped reduce fat and weight gain.  Hope Arcarbose does same because here in spain 40 50 mg tablets only cost $2.  Metformin is also super cheap

Van This research says that metformin and acarbose work well together. I just read the abstract.  I wonder if it is a matter of dosing and introducing the two together gradually.  I have acarbose on the way and have been taking metformin for the last 17 months.

David H Maybe, I just had a bad reaction first time I tried them together.  They really work in different ways, acarbose blocks the glucose from being absorbed into the blood stream, and Metformin lowers the glucose that is in the body.  I have been taking 2,000 mg of Metformin daily for past 1 year.  Has helped me lose 20 lbs., but I am at 165 lbs now and that is ideal for me, and had planned to cut back on Metformin anyway.  Once treatment works then it is prudent to reduce dose.  Did the same with my B/P which went down with the weight loss.  Was taking 20mg linsopril, now on 10 mg.  Only thing I do not reduce is the Rapamycin.  Let us know how your experiment goes with the combination.

David H Everything I have read says it takes a while for your body to adjust.  So small dose at first.  I take 500 mg Metformin at night and morning, then 1 Acarbose 50 mg during the day with 3 meals.

Hi Van , I'm from Tenerife and looking for Rapamycin. But seems Pfiser is doing big business here. The only available choice is Rapamune at astonishing 300€. Sirolimus is N/A.
Can you recommend me a Rapamycin supplier ?
      Graciasss!

Guillermo We live in Spain where the Customs are very strict.  So I and others have devised different strategies to get rapa because in reality it is a matter of life and death.  I and others order Sirolimus powder, Rx grade from China on Made in China website.  Many companies, but one good one is WuHan Hengheda  Pharm Co.,Ltd.  The cost is 75 euros gram + 50 euros Fedex shipping. (best to order at least 2 grams to ally shipping cost)  2 grams is 3-4 years supply for most.  The reason for powder is that a few grams of rapamycin fit in a business size envelope to get past Customs.  Also, shipping via Fedex helps make it look like important papers or samples for businesses.  Steve Roedde on this forum has checked the powder and it passes for Sirolimus.  If you will search on this forum you will find many posts by myself and others like Steve on the proper mixing and use of Sirolimus powder in your anti-ageing experimentation.  Good Luck
